Oh, oh...

I had undiagnosed, untreated whiplash somewhere back in the early eighties. They had me move my head through a range of motion, said, "Yup, you have a still neck." Gave me some valium and sent me on my way. The next morning I remember that I couldn't lift my head to get out of bed. About five years later, the chronic headaches started. Everyday, without fail. When I finally sought treatment films revealed a reverse curve in my c-spine. All these many years later I still have a fairly straight c-spine with just a hint of normal curve.

For about the last 5 weeks, I have had localized muscle aches and spasms along the right side of my neck annoying enough to interrupt my sleep several times each night. Monday morning I went out to feed and slipped on the icy packed snow, I didn't fall, but did a sort of chicken strut with my neck. By Tuesday morning, my entire neck was fairly stiff and sore. The case I posted about on Tuesday started at 1500 and we finished around 2200. Still I was no worse for wear. Wednesday had morphed into a full day, starting at 1100 with a laparoscopic right colectomy and ending at 1930 with an ORIF of a left ankle. The entire day my neck was killing me. Range of motion was now greatly decreased and pain and stiffness greatly increased. A "helpful" rad tech running the C-arm, poorly I might add, decided to adjust the OR light to help us see better. Unfortunately he is about 4 ft tall ( I am 6' o) and drug the light down to his level so as I stepped away from the field to switch places with the surgeon, of course I rammed the light with my head sending the jolt right down my already tortured c-spine. Lucky for him I was scrubbed in because my impulse was to pick him up by his neck and toss him across the room. I grabbed the light and sent it up to my arms length and focused it on the field from there. I made a very theatrical production out of it so that maybe he would notice? No, about ten minutes later the little bastard did it again! This time, at least I was aware of it. I ended the night with a raging neck ache and now unable to determine if the pain was muscular in nature or skeletal.

Thursday I had only one case at 0900 and we were done by 1030. My neck still screaming in agony. I had some errands to run before I could go home. I purchased a soft cervical collar. It hurts to hold my head up. One naproxen sodium the day before didn't touch the pain. One Daypro (okay they are about two years expired) the next day gave me about 1 hour of barely noticeable relief. When I got home I alternated ice and heat, ice bringing much greater relief than heat. I took two, 200mg, ibuprofen when I went to bed. I managed to sleep until 430 am when my neck would not quit screaming. I took two more ibuprofen and fell back asleep until 0930. Thank goodness I don't have any cases today.

The pain is a little more localized but no less intense this morning. It starts at the occipital on the right and travels all the way to the right shoulder at about the level of C-7, T-1. With a frightening trigger point at about c-3-4, or 4-5. My range of motion is limited to flexion and extension with discomfort. Lateral flexion is extremely limited more so to the left, and rotation to the left or right is barely a few degrees.

The good news is that my (self) diagnosis is most likely cervical strain. The differential DX is cervical spondylosis or herniated cervical disk. Regardless, both may be treated conservatively. I doubt either of the latter because I have no neurlogic symptoms in my arms, though I'm not sure how to give myself a neurologic exam ( the more I dwell on it the more I imagine weakness in both arms). I may have reached a therapeutic dose of ibuprofen as the two I just took seem to be working fairly well.

The bad news is that acute pain may take up to one week to subside I have a full week next week. I need to get hay today. Our Christmas party is tomorrow night. I have a three thousand dollar deductible I just as soon not use until 08. I have a dying dog who needs daily meds and two hungry horses so I can't do hospital time. I have no established PCP. I am a self confessed BAD patient.

I find it very frustrating and annoying to be in this position. I can't work on muscle relaxants, can I? Are there any varieties that would not impair my function? If I can just complete next weeks surgical obligations, the following week is the week of Christmas and so far nothing is scheduled, I only have a shitload of call. Perhaps I can heal. Or better yet make to 2008 when I can commit to some down time and that 3k deductible.

Make Mine, I had an Anterior Cervical Fusion, C6-7, several years ago. Numbness in my left index and long finger from the PIP out, weak triceps (could not push open a door or pull well on a retractor), extreme pain down my left arm (esp my shoulder joint which did not change with motion of the joint), burning sensation to the skin over my left upper arm, and felt better when I rested my arm on my head. :)

Every now and then I get "knots" in my neck/shoulders now which I ease with an Icy/Hot pad. I don't (and didn't then) take pain meds or muscle relaxants. If you can get someone to do a gentle massage that will help too. Best wishes.

Thanks. I am puzzled because I don't have any arm numbness, tingling.
Last night the pain was too intense. Muscle spasms driving me to tears. I have a script now for flexeril (10mg generic)which give very little relief. I can't sleep and the right side of my neck is still in agony. I may have to go back to ibuprofen but up the dose to 600 mg q 4hr? 6 hr? I have seven cases next week. Two with a new orthopod I just picked up. I just want to make it through the week!

Going to speak to neurosurgeon on Monday. I'm hoping some steroids will do the trick....enough to get through this next week. I am getting shooting nueralgia over the occipital up to the crown of my head, and now to the side of my face occasionally. I'm up to 800mg of Ibuprofen every 8 hr but it dulls things from about an 8 to a six, with breakthrough pain at about 3-4 hrs. Everything is down the right side of my neck only, the left side is fine!

My recommended musculoskeletal cocktail is typically ibuprofen 10mg/kg every 6 hours, either Flexeril or Skelaxin (less sedating) TID, and Vicodin 1-2 every 4-6 hours as needed for breakthrough pain (when off work) along with local heat and massage.

I hate the insurance deductible issue, but one way to look at it is that if things get bad enough to make you miss some shifts, then spending money now to possibly prevent that is ultimately worth it.

After I hurt my back, the physical therapy was more effective than any medication, but it was expensive. The ultrasound gizmo and the electrical stimulator worked wonders for me.

Thank you Scalpel. The severity seems to have lessened...it's been 8.5 hrs since my last 800mg dose and the pain is not as unbearable as it has been. In fact, other than the intense intermittent creeping neuralgia things are feeling better. Trying to hold out as long as I can so the next peak of relief will be during sleep. Also need to time following dose with cases tomorrow.
Have some vicodin from an old script (only took one after knee scope and even then it was for the spinal headache(of course it didn't work))but was afraid to take it last night. Maybe tonight.
The pain has been enough to make me give a hoot about the deductible, I'm at the point that I just want relief!!
I'm also hoping for physical therapy.

Here's my prescription: Massage by very handsome, single, hetero young man...440 mg naproxen sodium q12h x 2 weeks, long hot showers and PT if at the end of two weeks you still hurt.

Whatever you do, DON'T go to bed. The more you move, the faster your recovery will be, but do take it easy. No more headbutting operative lamps!

Fat Dr. Like your massage perscription. Know where I can get it filled?

I am definitely opting for P.T. over surgery.
I worked today and it felt like good therapy. The incessant, shooting, traveling cranial nueralgia is DRIVING ME INSANE!!!!Somebody please....make it STOP!!!

Yep, I have the same thing. Same X-ray, too. Went through physical therapy for it:

-Massage.
-Ultrasound.
-Heat.
-Light exercise to strengthen the trapezius. The weights? Regular-size soup cans. Do not overdo or you will trigger this again. No killer workouts. No serious lifting of any kind while this is sore.
-Note: NO ICE. You may feel better, but you're reducing blood flow to muscles that are acidic. Not good.

I was also given Flexeril, but that put me to sleep.

What you will learn over time: to not move your neck so much as you have in the past. To not stretch first thing in the morning. You may have to sleep with a small pillow to keep your head from flopping to the side. Do not sleep on your stomach. Avoid sitting in drafts. Do not shiver.

I even learned how to navigate my hometown making only right-hand turns when I couldn't turn to the right far enough to be safe making a left.

Then don't worry too much. Until there's numbness or tingling, it's just the muscles and ligaments acting up in response to the poor skeletal alignment.

Thanks for the advice.
I have to stay in the slow lane on the interstate because I can't safely do a lane change to the right.
I also have to drink out of a straw if I want to see the bottom of any drink.

Yep, that sounds about right! It's an unpredictable thing. I've put my neck out brushing my hair, brushing my teeth, and flossing.

Natural selection is gunning for me, I'm sure.
